Social determinants of health are the conditions in which people are born, grow, live, work and age that affect their overall health. For example, food security, transpiration, income, housing and safety. We are quickly learning that an integral part of healthcare delivery is to understand the space outside of the regular wellness visit, a nutritious diet and annual blood draws. As an IT team, we partnered with our community and social impact leaders to assess, build, align, and implement a new process to screen for Social Determinants of Health.
