This session describes the transition from paper-based Rehab therapy History intake forms and patient reported outcome measures (PROMs) to an electronic workflow using MyChart. We will outline our process for identifying appropriate questionnaires and visit types, as well as our approach to conducting phased pilots. The presentation will highlight build strategies leveraging Epic’s foundation rules for capturing core PROMs prior to the first evaluation and during follow-up visits, review completion rates, and discuss challenges encountered with therapist workflow integration.
