In rural healthcare settings, Emergency Departments (EDs) often serve as the front line for behavioral health crises, yet they face persistent challenges in care coordination, timely intervention, and regulatory compliance. This systemwide initiative within a rural academic health system introduces a comprehensive Behavioral Health (BH) Case Management model designed to streamline care delivery and improve patient outcomes. Key strategies include the implementation of a standardized ED Psychiatric Services consult order to reduce gaps in care, simultaneous notification protocols to ensure early engagement of referral specialists, and the integration of a standardized BH clinician assessment into EPIC for ease of documentation and clinical workflow. Additionally, targeted efforts to eliminate workflow redundancies have enhanced operational efficiency and supported regulatory compliance. This presentation will highlight the design and deployment of the model, share early performance indicators, and explore its potential for replication across similar rural academic systems seeking to strengthen behavioral health access and continuity of care.
