Description
Interpersonal Safety (IPS) is a critical Social Driver of Health (SDOH) and a required component in many Value-Based Care contracts, including ACO REACH. This session will explore how UNC Health has developed and implemented tools to integrate IPS screening into ambulatory primary care workflows. Presenters will highlight the use of Our Practice Advisory’s (OPAs), AgileMD pathways (Pathways@UNC) to guide responses to positive screens, and Epic-based tools to support clinical teams. Additionally, we will share how UNC Health uses Slicer Dicer to monitor IPS screening rates and trends over time, offering practical insights for health systems looking to scale similar initiatives.
