Living with diabetes can be challenging, however Diabetes Self-Management Education and Support (DSMES) is well known to help. Unfortunately, DSMES is underutilized in the current healthcare setting in part due to low rates of referral by healthcare providers. The Prompt [PROmoting Diabetes education and Management through Peer support and Team referral] study focused on increasing referral rates using OPA functionality.
