Description

Diabetes affects 38 million (11.6%) Americans and leads to significant health complications from cardiovascular, renal, and neuropathic disease. Early diagnosis and management is key to preventing morbidity and mortality. Health inequities exist in diabetes care. Approximately one in four Americans do not know they have diabetes. We will present data on a silent Our Practice Advisory (OPA)  for identifying patients with random glucose levels significant for diabetes (over 200 mg/dl) with no evidence for diagnosis of diabetes in the electronic health record (ie no billing or problem list diagnosis and no medication for diabetes). We will launch an active OPA in December 2024 to alert and provide clinical decision support to ordering providers of abnormal glucose and potential for diabetes. We will present the effectiveness of this OPA to improve diagnosis of diabetes in patients who are undiagnosed and also show our investigation of whether there are disparities in diagnosis by race, ethnicity, or gender.
