The use of restraints across acute care hospital settings is highly regulated at federal and state levels and requires scrupulous documentation. The Epic Foundation system includes various features to facilitate restraint documentation. However, compliant documentation still relies heavily upon provider and staff education, manual chart auditing, and an overly complicated clinical documentation workflow. Through an interdisciplinary review of the Duke University Health System’s (DUHS’s) restraint policy and clinical workflow, we sought to leverage opportunities within Epic to improve our provider and staff experience with restraint documentation and to minimize risk of compliance deficiencies. First, a single, rule-based restraint order panel with built in clinical decision support was built to facilitate ordering of the correct restraint type and to provide ongoing, real-time restraint education. To ensure compliance with the mandatory face-to-face assessment, this documentation was integrated into the violent restraint order as order-specific questions, which allow for report-based auditing. Additionally, the restraint flowsheet template was optimized to streamline documentation and resolve documentation discrepancies.
