Value-based care programs often rely on Epic’s HEDIS model to evaluate performance against payer contracts and identify patients for outreach. While this contract-based approach is valuable, our organization sought to broaden the lens: instead of limiting measurement to patients in value contracts, we wanted to understand how our entire Advocate Aurora population was performing against HEDIS specifications—particularly the Transitional Care (TRC) measure, which captures patient engagement after discharge.
