The NH Nursing Documentation Burden Reduction project was initiated to address the excessive time inpatient nurses spent documenting in flowsheets, which averaged 89 minutes per shift, thereby freeing nurses for direct patient care time.
In alignment with the Assessment/Reassessment Policy and the principle of Documentation By Exception (DBE), the initiative aimed to streamline documentation practices and enhance nursing efficiency. In collaboration with Clinical Practice Specialists, Informatics revised the Within Defined Limits (WDL) definitions, removed normal custom list options, and introduced system-wide macros. Enhancements included the addition of Rover flowsheets and mobile macros to support documentation on the go. These changes have led to improved compliance with DBE standards, reduced documentation time, and increased nursing satisfaction and time for direct patient care.
