The 2025 Physician Fee Schedule included new G codes for Advanced Primary Care Management (APCM). These new codes required the development of a new workflow to support patient consent, number of chronic diseases being managed, management of monthly charges, and methods to track patient participation over time. Development of a workflow to support across the continuum of care required the collaboration of multiple groups to achieve success. Program development will be discussed in detail including the workflow and the process design employed for APCM. Program metrics and lessons learned will be discussed.
