Implementation of OPAT (Outpatient Parenteral Antimicrobial Therapy) into clinical workflows has eased the burden of interdisciplinary care with the integration of a post-discharge antimicrobial therapy plan into discharge documentation. Another advantage in using OPAT is that it allowed us to utilize the CPP (Clinical Pharmacist Practitioner) to manage OPAT patients under the Infectious Disease clinic physicians.
