In Our current state, custom provider notes within Epic contain multiple redundancies that are present in the chart and result in extremely long notes as well as inconstancies on where to find pertinent information. This causes increased cognitive burdens on both providers and staff to review these notes. Evidence based research has shown a reduction in physician burnout and increased efficiency in providers with shorter, consistent notes.
