Clinical encounters have shortened in duration while the complexity of needed information has increased. Capturing relevant data ahead of time can improve the experience for both patients and providers, making visits more focused. In ECU Health’s new Lifestyle Medicine (LM) Clinic, a custom intake form designed by the American College of Lifestyle Medicine (ACLM) was built as an Epic questionnaire. This form not only collects nationally standardized baseline LM data but also helps identify specific goals and expectations of the patient. By understanding patient-specific needs before the visit, providers can offer a more focused and patient-centric experience. We implemented changes that sifniuficantly increased the completion rate before the appointment from 20% to 40%.
