                                    As more health systems screen patients for health related social needs (HRSNs), integration of Social Health Access Referral Platforms (SHARPs) become a common solution to connect patients to resources. SHARPs are designed to connect patients to social and behavioral resources and provide outcome data on whether the referral was accepted and successful. The Closed Loop Rate (CLR) offers data on number of referrals accepted and closed but knowing that the patient received an actual service (Successful Connection Rate (SCR)) is of greater interest and value to the referral sender. When implementing SHARP, governments, payers, and health institutions should consider measuring the SCR with the CLR.
