A successful program to gather Social Determinants of Health (SDoH) data requires the development of an efficient workflow and alignment with organizational imperatives. Novant Health launched Food Insecurity screening as a non-productivity compensation metric for physicians and began gathering additional SDoH components in 2021. Now in the third year, this program continues to evolve to bring value to patient care. Results from 2023, goals for 2024, and the change process will be discussed in detail.
